Late Policy

I aim to start and end appointments at the agreed-upon time, out of respect for both your and my time. Please login to 10 minutes before your appointment time to be sure we can start the appointment on time. If you do show up late, we will discuss your options. We may need to have a shorter appointment than you would have had otherwise, and be limited to discussing only your most pressing issue, however you will be billed for the full planned-on time. Alternatively, we may decide we need to reschedule for a different day. If this happens, you will be charged the cancellation/no show fee.

Cancellation/No Show Fee

Please cancel your appointment online at least 24 hours prior to your appointment if you need to cancel. This will allow time for your appointment slot to be filled by another patient. If you fail to show for your scheduled appointment, or did not notify the office within 24 hours of your scheduled appointment time, you will be subject to a “No Show/Cancellation” fee of $50. In the event of an actual emergency in which prior notice could not be given, consideration will be given and a one-time exception may be granted. Deep Roots Family Medicine and Be Well Holistic Health have their own cancellation/no show fees.



During your telemedicine visit with Dr. Lily, she will be in a private room and can assure your privacy from those around her. However, it is up to the patient to hold the call in a private area on their end.


Primary Care

Dr. Lily does not serve as a primary care provider (PCP) and asks that you have a PCP on your care team.


For a life-threatening emergency, always call 911 immediately.


Communicating via the ChARM portal is the best and most secure way to contact Dr. Lily. You can message Dr. Lily to clarify a treatment plan, discuss payments, scheduling etc. If your question is about a new medical issue that was not discussed during a visit, or is extensive, please schedule an appointment to discuss it completely.


Email can be a convenient way for us to communicate, however we prefer you to use our patient portal for most questions. Please be aware that email is not a secure form of communication. For these reasons, it is best to use the patient portal through ChARM for questions pertaining to medical concerns. Email is more appropriate for scheduling and payment questions.

For questions that cannot be resolved via email or the portal, you can call Dr. Lily. This voicemail will be checked once daily Mon-Thursday, and she will return your call as soon as she is able. For urgent medical questions, please call your PCP or seek urgent care.

Termination of the Doctor-Patient Relationship

You may terminate our relationship at any time for any reason. Communication from you expressing your desire for termination is appreciated, but it is not required. Likewise, Dr. Lily may terminate your relationship at any time. Generally, she will reserve this measure for patients who are not abiding by the stated policies, are delinquent in paying bills, or are disrespectful.  If she chooses to terminate your relationship, she will notify you in writing. You will then have 30 days to find a new physician, during which time she will be available to you for urgent health issues only. You may request a transfer of medical records to your new physician.
